Medical Insurance

Dental Insurance
The district offers a PPO dental insurance plan through Blue Cross Blue Shield of Illinois. Click below for helpful links and claim forms. You may contact BCBSIL Dental Services at 800-367-6401.
Vision Insurance
The district offers a vision plan through VSP Choice. This is an optional plan available to both PPO and HMO participants. Please note that our PPO medical plans include a vision exam for a $10 copay through VSP. HMO participants are eligible for a free vision exam each year provided through EyeMed.
Prescription Coverage
PPO prescriptions are covered by Express-Scripts. HMO prescription coverage is administered by Prime Therapeutics under their medical plan through BCBS. PPO participants must use their Express-Scripts insurance card, not their medical insurance card, for prescription medications. HMO participants will use their medical card. Flexible Spending Accounts

Supplemental Life Insurance
The district offers optional life, spouse life, child life, and AD&D insurance through Voya. For elections over the guaranteed issue amount, employees must submit an Evidence of Insurability (EOI) form directly to Voya. The additional amount will not go into effect until it has been approved by underwriting at Voya. You may contact Voya at 800-955-7736.
