All students entering a school bus or building most have a completed, signed pre-certification form. Forms and lanyards are provided by the student’s school. If you do not have forms, you may print one by clicking here. Please contact your school office to receive additional pre-certification materials.
As a reminder, IDPH exclusionary guidelines have changed and abdominal pain, nausea, runny nose or congestion no longer require isolation.
